Yes. Formstack's acquisition announcement, dated December 12, 2018, says "Launched in 2011 by Tishan Mills and Ravin Shah, QuickTapSurvey services more than 180,000 customers in 175 countries." Tishan Mills, writing for the University of Waterloo alumni site on June 23, 2017, refers to "my co-founder Ravin Shah (BMath '02)".

In a Notable Life interview published September 10, 2014 he said "I'm the Co-CEO and Founder of TabbleDabble, a tech startup that created the world's highest rated survey app, QuickTapSurvey", and that he started it "with my good friend Tishan Mills in 2010". In a Sampford Advisors interview dated June 28, 2018 he said "I was definitely more geared towards the sales and marketing aspect of the business – anything to do with client success."

What did he do before QuickTapSurvey?

Technology consulting and finance. Sampford Advisors' June 28, 2018 profile says Shah and Mills met as roommates after enrolling at the University of Waterloo in 1997, graduated in 2002, and that Shah "moved to Chicago and started working as a consultant for Accenture", then worked at Lehman Brothers for a year before returning to Toronto.

What happened to QuickTapSurvey?

Formstack bought it. Formstack's announcement of December 12, 2018 reports the acquisition. A year and a half earlier, Mills wrote on June 23, 2017 that the founders had "bootstrapped our company to profitability with no debt or outside investment" and had a team of 35 in downtown Toronto.
